Our Palliative Care Outreach Team of specialists provides personalized medical and emotional supports for people and family members, in the comfort of your home.

If you are having trouble managing pain and other physical symptoms of your illness, we can help. Or perhaps you or a family member would like to talk with one of our counsellors for some emotional support.

We listen to you, answer questions and understand your needs and what’s most important in your life. 

Our team is a partnership with you, your doctors and other health care professionals. 

Everyone works together to help you.

If you are having trouble managing pain and other physical symptoms of your illness ask your family doctor or Care Coordinator if a referral to Hospice Niagara’s Palliative Care Outreach program would be beneficial.

Maybe you’d like to talk about a difficult topic, worries and fears, anxiety or feelings about your diagnosis or the future. 

Our counsellors have training in palliative care, crisis support, grief and loss, the dying process and much more. 

It can be helpful to have a conversation with a non-judgmental, confidential and compassionate professional.

We offer supportive counselling for family and friends.

The team is made up of palliative care specialists who meet one-on-one with you in your home and who also provide expert advice and consultation to other health care professionals involved in your care. 

They could include: palliative care physician, nurse practitioner or registered nurse, a psychosocial bereavement counsellor and a clinical navigator.
