This program can support patients by providing a palliative approach to care early in their illness which can improve health outcomes and quality of life. 

A specially trained palliative care nurse, in partnership with primary care providers use remote technology to help individuals monitor their health. This could result in timely response to symptoms of serious illness.

Patients report their symptoms via a simple survey on a mobile device.  Responses are sent to a dashboard monitored by a registered nurse dedicated to this program. Depending on the issue identified, the RN can support the person with a phone call and/or link the patient to the most appropriate health care resource or primary care provider. 

The Virtual Care Monitoring program is provided to family health teams and patients at no cost. To access the virtual care monitoring program, ask your primary care provider for a referral.
